C. Special/Hazardous/Toxic Waste Generating Establishments

For agricultural/poultry supplies, paint trading, hardware/construction supplies, hospitals, clinics and laboratories.

• All documents needed for new applicants and renewal
• Documentation report and proof of proper disposal of special/hazardous/toxic wastes (not collected by regular garbage collectors)

Endorsement for Securing Permit to Cut

• Request letter of the owner of the trees
• Barangay Clearance/Endorsement of Punong Barangay where trees to be cut are located
• Copy of Tax Declaration/OCT/TCT
• Updated receipt of tax payment
• Certification from Municipal Agriculture Office for fruit-bearing trees and coconut
• Business Permit of chainsaw that will be used for cutting

Endorsement for Securing Permit to Transport

• Consent letter from the owner of the trees cut
• Endorsement of Punong Barangay where trees to be transported are located
• Copy of CR/OR of the conveyance/vehicle or Transport Agreement if the vehicle to be used is hired/rented
